All Delta Academy students must meet school immunization requirements to enroll and remain in good standing.

Who this applies to

• All students enrolled at Delta Academy (on-campus and online).

• New students and returning students each school year.

Due dates & compliance

• Ensure you have provided Delta with proof of required immunizations.

  • You can securely upload at the bottom of the page.

• Students out of compliance may be unenrolled until records are updated.

• If you’re not sure which immunization is causing the compliance issue, please try to confirm with your child’s provider or the school nurse (Becky.Swaney@deltaacademylv.com).

Exemptions

Delta Academy honors exemptions as permitted by state guidance.

Religious Exemption

• If you choose not to have your child receive one or more required immunizations for religious reasons, please complete the Religious Exemption Form.

• This form must be completed each school year (valid for one academic year only).
